Affiliation Requirements
- Health Certificate: where a doctor certifies that you are in good health condition to perform physical activities or begin an excersise program.
- Felony Clearance Certificate: must be recent, no more than thirty (30) days old, (do not apply to legal age minors).
Affiliation form and Payment: in addition to the above mentioned documents, you must completely fill and sign the Affiliation form to the AIKIDO program and make your Fee payment before start practicing AIKIDO.
